• Describe their contributions to the community, either as a volunteer or by exceeding the normal requirements of their paid work. Include community groups or professional bodies they have been involved with.

If you are nominating a young person tell us:

• How are they participating in their community?
• Describe how they are active in youth issues.

If you are nominating a community group:

• What are the contributions the group has made to the community? What type of community service projects has the group been involved with that has benefited the community?

If you are nominating a community event or project:

• Tell us about the event or project.
• If it was an event, when was it held and who attended? What were the highlights of the event and how did it benefit the community?
• If it was a project what and who was involved, what was the outcome and benefits to the community?
